내분비 외과 의사로서 갑상선암 및 갑상선기능 항진증, 부갑상선 기능 항진증 및 부신 종양 등에 대한 수술적 치료를 수행하고 있으며, 자연스럽게 주로 이와 관련한 연구들을 진행하고 있다. 이 중 주요 관심 연구 분야는 크게 두 가지로, 갑상선 수술 중 손상이 발생할 수 있는 후두신경의 기능 보존을 위한 수술 중 신경 모니터링 (intraoperative neuromonitoring, IONM)에 관한 연구와, 흉터 없이 통증을 최소화하여 정밀하게 갑상선 수술을 시행할 수 있는 로봇 경구 갑상선 절제술 (transoral robotic thyroidectomy, TORT)의 개발 및 이에 관한 연구를 지속적으로 수행하고 있다.

1) 로봇 갑상선 수술
 로봇 갑상선 수술은 3D 시야의 다관절 로봇팔 수술장비를 이용하여 내시경 수술보다 한층 정밀하고 안전한 수술을 시행하는 방법이다. 그 중 김훈엽 교수가 세계 최초로 개발한 독창적인 로봇 경구 갑상선 수술(transoral robotic thyroidectomy, TORT)은 입 안을 통해 수술용 로봇을 넣어 갑상선을 수술하는 방법으로, 수술 부위가 전혀 노출되지 않고 아예 흉터가 남지 않는 획기적인 수술법이다. 수술 후 한 달 정도면 입안 수술 상처가 사라지며, 수술 후 통증도 기존 로봇 갑상선 수술에 비해 확연히 적을 뿐 아니라, 가장 근접한 부위에서 정밀하게 갑상선을 절제함으로써 수술 후 목소리의 변화도 거의 없어 환자들의 만족도가 높다. 진정한 의미의 갑상선에 대한 최소 침습 수술(minimally invasive surgery)로도 인정 받고 있다. 이 수술법은 기술적 완성도 및 임상적 안전성을 세계적으로도 인정 받아, 미국 존스홉킨스대학병원과 클리브랜드 클리닉 등 세계 유수의 병원에 수술법을 전수했으며, 이 뿐 아니라 미주, 유럽, 아시아 각국의 의료진이 앞 다투어 수술방법을 전수받아 가고 있어 국제적으로 의료한류를 이끌고 있다.
 김훈엽 교수는 여기에 그치지 않고, 현재 진행성 갑상선암 환자의 치료를 위한 로봇 경구 경부 곽청술 (transoral robotic neck dissection) 술기를 개발 중이며, 로봇 경구 갑상선 수술에 최적화된 새로운 수술용 로봇 시스템과 관련된 수술 기구를 개발하기 위한 연구를 지속하고 있고, 이와 관련하여 수술용 내시경/복강경 청결 유지 장치 및 관련 기술에 관한 특허를 다수 출원하였다.

2) 수술 중 신경모니터링
 갑상선 수술 중 신경모니터링법(intraoperative neuromonitoring, IONM)은 갑상선 수술 중 손상받기 쉬운 후두신경에 대해 전류 자극을 통한 근전도를 확인함으로써 수술 후 환자의 목소리를 완벽에 가깝게 보존하는 새로운 수술기법이다. 수술 시 예상치 못한 신경의 주행경로 확인 및 등에도 사용될 수 있어 이로 인해 발생할 수 있는 신경손상 위험도를 낮추고, 수술 중 예기치 않은 신경손상 여부를 확인할 수 있어 현재 미국, 유럽 등의 의료 선진국에서는 대부분의 갑상선 수술에 사용되고 있다. 김훈엽 교수는 국내에서 선도적으로 이를 수술에 도입하여 기존의 절개 수술법에서부터 로봇 갑상선 수술에 이르기까지 광범위하게 적용함으로써 수술 후 신경손상으로 인한 합병증을 획기적으로 낮추었으며, 특히 로봇 경구 갑상선 수술은 후두신경을 정밀하게 잘 확인할 수 있는데다가 이 신경모니터링을 병합함으로써 완벽에 가까운 수술 후 목소리 보존을 보이고 있다. 또한 김훈엽 교수는 이 신경모니터링법으로 확인할 수 있는 후두 신경의 병태 생리와 손상 기전 등을 동물 실험 및 임상에서 규명하기 위한 연구를 지속하고 있으며, 이와 관련한 수십 편의 SCI급 논문을 저술하였다. 특히 해당분야의 선도적인 외국 연구자들과 공동 연구를 활발히 진행하고 있으며, 이로 인해 최근 국제신경모니터링연구회(international neural monitoring study group, INMSG)의 운영 위원으로 임명 되었다.
